If you wish to move a phone number from an existing provider to Evolve IP, you will need to complete an LOA (Letter of Authorization). Use the forms below to complete your LOA. You will be required to provide it to a member of Evolve IP during your implementation or by submitting a ticket through the Evolve IP Help Center.
Docusign WebForms
Below you will find several links for Docusign Webforms. These forms allow you to fill in the LOA in an easy walk through method. Each field contains descriptions and explanations on how you need to fill it out. Upon completing the WebForm, a Docusign link will be emailed to you to review the completed document and sign it. Once signed, you will receive a copy and it is up to you to submit it to a member of Evolve IP for official submission of the port. Completing a Docusign LOA does NOT automatically submit your LOA for porting.
